Assumption of Risk:
I understand that engaging in sports and physical activities involves inherent risks, including but not limited
to the risk of personal injury, property damage, or even death. I voluntarily assume full responsibility for
any risks of loss, property damage, or personal injury, including death, that may be sustained by me or my
property, whether caused by the negligence of the releases or otherwise, while participating in any activities
at AZ Soccer Pro.
Release and Waiver:
I hereby release, waive, discharge, and covenant not to sue AZ Soccer Pro, its owners, directors, officers,
employees, agents, and assigns (collectively, the "Releases") from any and all liability, claims, demands,
actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including
death, that may be sustained by me, or to any property belonging to me, while renting or using the sports
facility, including any injuries caused by the negligence of the Releases.
Indemnification:
I agree to indemnify and hold harmless the Releases from any loss, liability, damage, or costs, including
court costs and attorneys' fees, that they may incur due to my participation in said activities, whether caused
by the negligence of Releases or otherwise.
Medical Treatment:
I consent to receive medical treatment deemed necessary if I am injured or require medical attention during my
participation in activities at AZ Soccer Pro. I understand that I am solely responsible for all costs related
to such medical treatment, medical transportation, and/or evacuation.
Use of Facility:
I agree to comply with all rules and regulations set forth by AZ Soccer Pro regarding the use of the facility.
I understand that failure to comply with these rules may result in the termination of my rental and
participation privileges without refund.
Insurance:
I understand that AZ Soccer Pro does not carry or maintain health, medical, or disability insurance coverage
for any participant. As such, I am encouraged to have adequate health and accident insurance coverage.
Photographic Release:
I grant to AZ Soccer Pro the right to take photographs and videos of me in connection with my participation
in activities at the facility. I authorize AZ Soccer Pro to use and publish the same in print and/or
electronically for lawful promotional purposes.
Governing Law:
This Waiver and Release shall be governed by and construed in accordance with the laws of the Province of Ontario.
I have read this waiver and release of liability, fully understand its terms, understand that I have given up
substantial rights by signing it, and sign it freely and voluntarily without any inducement.
